Medical Plan
The Deaconess Medical Plan, offered through Anthem, is a preferred provider organization medical benefit plan. It covers medically necessary expenses for physician, hospital, surgical and therapy services as well as prescription drugs.

For verification of eligibility, benefits, claims status, provider network verification or other inquiries, please call Anthem at 1-800-552-9159.

Online Access
Hospital employees can access information regarding the Preferred Provider Organization's (PPO) preferred group of doctors, specialists, etc. on the Internet. When you visit a provider in this tier, the plan will pay higher benefit levels and your out-of-pocket costs are lower. To obtain a list of the participating physicians within this tier, please visit www.anthem.com.

Retirement Planning
Deaconess Hospital partners with MetLife in providing a 403 B plan to aid hospital employees in saving for their retirement. For more information on this plan, go to MetLife at http://mlr.metlife.com. Once you register, you will be able to pull up and review your account online.

MetLife representatives are in the hospital every Monday and are happy to meet with you to discuss the funds available through the plan. To make an appointment, call the Human Resources Department at 559-2414. You may leave messages for the MetLife reps at 559-2700; however, HR schedules their Monday appointments.

Corporate Compliance
The Corporate Compliance Program at Deaconess Hospital and Deaconess Associations Inc. helps all of us "do the right thing." Employees also continue to receive regular compliance refresher training because these issues are important tous, as individuals and as a corporation. Deaconess, the government and our customers all expect Compliance, and it the basis of quality care and service.

However, if you feel uncomfortable going to your supervisor and want to contact the Corporate Compliance Officer directly, you may contact the DAI Corporate Compliance Officer with concerns, questions or a report of possible wrong-doing.

When contacting the Corporate Compliance Officer, please provide the following information.

  1. Your name and how to contact you. (Remember, you can also make an anonymous report. But if your report is anonymous, it is even more important to include key details so that a thorough inquiry can be made.)
  2. The name of the DAI business involved. (For example, I work for XYZ nursing home in what state.)
  3. Names (s) and position(s) of the individuals involved.
  4. What the individual did.
  5. Day of the alleged conduct or compliance violation.
  6. Provide as much detail as possible concerning where, when, what, how, etc.
